Self-referrals via: https://preventing-diabetes.co.uk/self-referral/

Or by filling in our Know Your Risk assessment online by visiting this website link: https://riskscore.diabetes.org.uk/start

Alternatively, phone our booking hub on 0333 577 3010. Our booking team are available Monday to Friday, 9:00 AM to 16:30 PM and Saturdays 9AM – 5PM.
To register a patient over the telephone, they need to have their NHS number, their most recent HbA1c reading and the date of the reading. Link workers can call to register on behalf of someone but we would need them to be with you to give permission first.
